Patient Pre-Registration
After making your appointment, please download and fill out a Patient History Form and Medical Questionnaire (Spanish Version). Filling out this information in advance is very important and will help make your check-in experience as quick as possible.
The Patient History Form will help us learn more about you, including previous treatment for medical diseases or conditions. It’s important for us to know which medications you currently take. List all prescriptions and over-the-counter medications, including dosage, and bring them with you to your appointment. The Medical Questionnaire (Spanish Version) will also help us learn more about risk factors such as a family history of diabetes or vision problems.
Also download and fill out the Authorization for Treatment & Financial Responsibility Form as well as the HIPAA Acknowledgement form.
Bring all insurance cards with you to your appointment. If you have been referred by another doctor, bring your referral copy, especially if your insurance carrier requires proof of the referral.
Please bring a copy of your most recent prescription (Rx) for glasses or contact lenses. Also be prepared to list your current medical doctors or health problems you have.
